Abstract
Four-dimensional (4D) printing has emerged as a transformative paradigm in additive manufacturing that uniquely incorporates time as the fourth dimension, extending static three-dimensional constructs into dynamic, stimuli-responsive architectures capable of programmed evolution over time. Unlike conventional 3D printing, which produces static geometries, 4D printing creates adaptive structures with programmability, temporal functionality, and reconfigurability that enable active interaction with their environment. This review critically synthesizes advances across seven interconnected domains: theoretical foundations, material innovations, fabrication modalities, biomedical translation, computational intelligence, reproducibility and ethical accountability, and sustainability with future directions. Stimuli-responsive materials, including shape memory polymers, liquid crystal elastomers, hydrogels, ceramics, metals, and cell-laden biomaterials, provide diverse actuation pathways, while fabrication strategies such as digital light processing, two-photon polymerization, direct ink writing, extrusion-based printing, and bioprinting translate these capabilities into functional constructs. Biomedical applications demonstrate translational potential, achieving > 90% cell viability and vascularization within seven days across tissue engineering, implantable devices, and drug delivery platforms. Recent advances in 4D-printed cardiac occluders, vascular stents, intestinal stents, and patient-specific surgical guides further underscore the clinical relevance of this technology. Computational intelligence accelerates design cycles via machine learning, topology optimization, and multi-physics simulations, achieving prediction accuracies above 90% and design cycle reductions exceeding 50%. Reliability and reproducibility frameworks safeguard scientific integrity, while ethical considerations, including informed consent, data privacy, and equitable access, ensure responsible biomedical translation. Sustainability is advanced through recyclable resins, eco-friendly energy devices, and bioinspired composites, while emerging cell contractile force-mediated actuation opens new frontiers in biologically driven morphogenesis. Despite these achievements, critical challenges persist in scalability, high-throughput biofabrication, and standardized evaluation frameworks-gaps that this review systematically addresses. By integrating adaptive materials, computational design, and ethical accountability, 4D printing redefines the boundaries of manufacturing and sets the stage for next-generation technological innovation in personalized medicine, sustainable infrastructure, and intelligent functional devices.
